i was able to get live chat about an hour ago. he told me in order to cash out $40, you have to have an available balance of $4,135.  Anyway, got it up to over $5,000 and want to cash out, but now I can't get live chat. 

finally got live chat about 5 minutes ago.  he said payout options are netspend, obopay, and account now. gave him last 4 digits of my card and email address. he said it takes about 3-7 business days for transaction to be completed.  I will post back when I hear something.
