I have a response to my query freedom4 and this is the reply:

The minus figures are because of high withdrawals she has had, the cashback is given on the Net Losses so we calculate the net win minus any bonuses which provides us with a figure, then if its $200+ they get $50, $60 or $75. I have double checked the Manhattan Slots the last bonus was given on the 17th June 2013, the players net losses are $747.98 minus bonuses of $713.48 so his current Net Losses are $34.50. I can’t see when the initial request is but I have double checked the account and I am sorry to say that the amount is correct, she does not qualify just yet.

I also wanted to comment on how they figure out whether you are entitled to the Cash back.  It should be if you have made deposits since your last claim and haven't won anything from those deposits then you should be entitled to the cash back.  It should not be based on how much money you have won over a course of time.  That is not right in my opinion.  If you made deposits and you lost them, why shouldn't you be able to claim them back?
